Bortezomib in Soft Tissue Sarcoma: Insights via Functional Testing
Advanced Soft Tissue Sarcomas (STS) are rare, diverse tumors with limited treatment pathways. Standard first-line anthracyclines yield only a 20–30% response rate, and genetic markers rarely guide therapy. Functional precision medicine offers a solution by testing live tumor response to identify effective combination therapy.
Bortezomib Repurposing in STS
Bortezomib, a proteasome inhibitor used in multiple myeloma, blocks the degradation of ubiquitinated proteins to trigger apoptosis. While its impact as a single agent in STS is limited, its repurposing as a combination therapy shows significant potential. Preclinical data suggest biological rationale in Rhabdomyosarcoma, Ewing Sarcoma, and Osteosarcoma.
The Optim.AI™ Methodology
The Optim.AI™ platform performs drugs sensitivity testing through a streamlined workflow:
-
Organoid Culture: Patient cancer cells are grown into 384-well organoids.
-
Ex Vivo Testing: Organoids are co-cultured with 12 drugs and all possible combinations.
-
Analysis: A proprietary platform ranks treatments by Normalized Cellular Viability (NCV); lower NCV indicates higher tumor killing.
Clinical Evidence and Results
In a study of six STS cases, five received an Optim.AI™-guided Bortezomib in Soft Tissue Sarcoma regimen. This resulted in an 80% clinical response rate with a median duration of 5.5 months.
Optim.Al™ successfully identifies patients who benefit from individualized, non-standard therapies, proving its utility in Soft Tissue Sarcoma (STS) treatment.
